Firm fined after worker falls through service riser shaft

A Derbyshire based firm has been fined following an incident in which a worker fell down a service riser shaft.

On 15 June 2015 a 36 year old Romanian worker was dismantling a temporary structure at a building under construction at Fletcher Development, De Montfort University, Leicester, when he fell 13 meters.

Following an investigation by the Health and Safety Executive (HSE) it was found that there was an unsafe system of work in place and the degree to which workers were supervised was inadequate.

After pleading guilty to breaching Regulation 4(1) of the Work at Height Regulations 2005, David Ashley Construction Limited, of Lydford Road, Alfreton, Derbyshire was fined £20,000 and ordered to pay costs of £1,776.
